Complaint Form: [ English ]  [ Spanish ]  If you have a complaint against a Ventura County Sheriff's Department employee, you have the ability to file a complaint form either in person, or by mail. Complaint forms are also available at our Headquarters (800 South Victoria Avenue), Camarillo, Fillmore, Moorpark, Ojai, and Thousand Oaks stations. After opening the form, print it out using your printer. Included in this file are instructions on how to complete the form and where to mail it, along with an overview of the process.

Employment Application: You may download an employment application into an Adobe Acrobat file. Instructions on how to obtain a free copy of Adobe Acrobat are listed at the bottom of this page. The application is part of the examination process. Failure to complete all items on the application will result in your elimination from the examination process. TYPE or PRINT CLEARLY in black (preferred) or blue ink. Complete all areas of the application. Resumes are not accepted in lieu of completing any portion of the application. Submit additional employment history pages only if necessary.
